Nursing process


Nursing process is a systematic, rational method of planning and providing individualized nursing care.
This holistic approach to the patient, the centre of the care plan is divided into five steps:
1-Assessment: gathering Data about the patient.
-Assessment involves taking vital signs (Temperature, respiratory rate, Heart rate, Blood pressure, and pulse & Pain assessment.
-Performing a head to toe assessment.
-Listening to the patient's comments and questions about his health status
2-Diagnosis: the health care provider will form a diagnosis about patient health problem (cause and effects).
3-Planning: the problem was discovered, so the nurse puts a care plan in order to help the patient to recover from his illness, and to restore his health.
4-Implementation: the care plan will be performed (medications, surgical procedures, positioning, taking vitals…) 5-Evaluation: after completion of the plan, the results should be evaluated in order to see the efficacy of this specific plan, and there is a need to edit it accordingly.
